APPLICATION FORM
All applicants must submit a reservation application form to the Administrator for consideration. Submission of an application does not guarantee acceptance to the Academy.

If you experience any difficulties with the online reservation form, you may use the pdf form. We suggest that you follow up on your online application if you do not receive an email acknowledgement of your form within 7-10 days.

There are two options in which to process your completed form.
1. Print a copy of the completed fillable form and fax to 520.466.2656.
2. Save a copy of the fillable form using the "save as" option. Email your application form to the administrator.

ACCEPTANCE

The Administrator will notify the candidate via email of acceptance to the Academy. The deposit must be received within 10 days of the notification.
 
RESERVATION DEPOSIT
A deposit of US$500 is required to guarantee a reservation slot. The balance of course fees and exam fees are due no later than the first day of the course.

Payments to be made by credit card require completion of the USAPR Credit Card Authorization form. This form should be faxed to 520.466.2656 after receiving confirmation of acceptance to the Academy.

RESERVATION CONFIRMATION
Upon receipt of the deposit, candidates will receive a reservation confirmation via email outlining requirements and preparation for the upcoming course.  

CANCELLATIONS AND REFUND OF DEPOSITS
Cancellations must be submitted in writing.
For a reservation cancelled more than 45 days prior to the start of the course, a refund of $350.00 will be made within 14 days. A course may be rescheduled (one time), and a fee of $150.00 will be charged if made 45 days prior to the start of the course. A course reschedule must be made within 12 months of the cancelled course.

NO REFUND of deposit for reservations cancelled less than 45 days before the start of the course.
 
DRESS CODE
USAPR has no dress code requirements. It is recommended that candidates wear suitable clothing and shoes for the climate and work environment (packing on the floor). Candidates may bring their own knee pads.

TRAVEL AND ACCOMODATION
Travel and accommodation costs are NOT included in the course fees. Candidates are responsible for their respective travel and hotel accommodations.  Make your reservations early.
 
ACCOMMODATIONS
         
There are many hotel rooms within a 20-minute drive of USAPR and Skydive Arizona, and some are within a 5- minute drive. Casa Grande accommodations are preferable as there are more restaurant and food stores in the close proximity to the hotels.
